Depending on: your age and duration of symptoms, this can be a worrisome sign.  Unfortunately, a change in stool caliber can be a sign of stricture/scarring in the gut and even tumor (for at risk individuals).  It could also be benign as in cases of constipation predominant IBS or transient infection with swelling of the gut wall.  If ongoing, discuss this with a physician.

: Increase water and fiber in your diet and with some exercises it would be easier to push out the waste. If there is high simple sugars in your diet you are likely to have low residue in the bowel and that could result in small bowel movement. It may just be that you have not been as active also is why you do not have the expected move of your bowel.

Timing of biopsy: Cervical biopsy should not bleed, after a couple of days. Bowel movement should not induce bleeding from the biopsy site. Whether it is cervical of intestinal, bleeding should not be ignored and you should contact your doctor for an evaluation.

Infant constipation: Most likely reason is formula intolerance. Please keep in mind it is not lactose intolerance, rather milk protein intolerance. Would recommend using  Gerber Good start, if no change in 2 weeks then need to go to Alimentum or Nutramigen.  
If symptoms still persist after these changes you may need to be seen by a specialist. Karo syrup is OK for short term. May also use baby fleets enema as needed

: I would just continue what you are doing. It takes ~4 days for the first med to make it through to the outlet in a constipated kid. Another day or two keeping fluids up and good nutrition &amp; he should do well.
