4 yr old tested positive for influenza! started tamiflu, (oseltamivir) day 2 seemed normal but spiked fever at night! what can i expect from here?

Tami-flameout: Tamiflu is just OK and not a miracle drug. It has been shown to slightly shorten the course and severity of the flu, but not in everyone. It doesn't have a lot of bad side effects in most people. You'll need to keep-up on the fluids and Tylenol (acetaminophen) and watch for worsening symptoms or change in behavior. If that happens, taken him (?her) back to the doctor.

Infection: There could be overlapping infection such as pneumonia.Flu.Colds.Ear infection.

Herpes zoster, MRSA: Sounds like your shingles rash has become infected with MRSA. If you're on an antibiotic for MRSA and it's starting to improve or not getting worse, just hang in there; it can take several days to clear up. Are you also on anti-shingles treatment, e.g. valacyclovir? If high fever or getting worse, call your doctor's office; another doctor will be covering for her. Good luck!

: Viral infection usually causes fever in the evening and at night. You can give her tylenol liquid in the afternoon and before bedtime. However, most viral illness will subside after 3-5 days so if symptoms persists past 5 days you daughter should be evaluated her pediatrician. Make sure she is taking fluid well during the day time.

Brief fever episode: Brief febrile episodes are fairly common in infants and children,  especially in 12-36 months age group.  Viral illnesses are most often the culprit.
In case his fever is associated with significant change in his general well-being, e.g., feeding, activity, or alertness, seeking medical attention would be prudent.

See below: Talk to your pediatrician, or go by an urgent care center. The ed will be packed and crowded and a simple cold will take a long time to treat. If the fever stays down, and the child is taking fluids you are probably all right for now, but if it continues do as above.

Get checked again.: Something is going on and it is certainly not normal, nor should it be ignored.  Get seen today, please or call your doc.The CT scan may have been done too early to show something clearcut and a repeat scan will show any changes.  So it is a good idea to go to the ER where the first CT was done, so a comparison can be made.

Observe: Rest, drink fluids and use Tylenol rather than Motrin. Consider COVID 19 screening, if fever lasts for more than 2 days.

Aspiration: Aspiration of any products, rice or anything else, can lead to the formation of aspiration pneumonia.  This type of pneumonia can be very serious and cause high fevers, coughing, chills, respiratory problems to name a few.
