Too little info: Diabetes requires multiple interventions. Diet and exercise are the two most important. If the diet is really  well controlled and your sugars are still high you need to track them. Then take that to your doctor who can adjust your dosing of insulin. DO NOT INCREASE INSULIN YOURSELF, unless you have a sliding scale from your doctor.

Long term htn: Your father has hypertension.  This causes long standing stress on kidneys and heart.  Now he has developed another heart problem which is not uncommon.  The medication may be related or it may be from the long standing problem of hypertension.  Your dad may need more frequent visits to the doctor during changes and additions of medication.  It is good that you are helping.

Fasting BS of 121 is not too bad for his age. 2 hour after meal BS should also be coming down to pre meal level.

my father fell while drinking and now has an open wound, about an inch open. it is not bleeding that much, but it looks like it need stitches. what should we do?

Well,: If you think he does need stitches you better take him to and emergency room asap as the window for primary suture is rapidly closing. If it is on the lower leg after stitching  have some compression stocking on the leg to offset possible swelling that will delay healing. Make sure to be given a tetanus shot too!
