Piles of trouble: Hemorrhoids may be thrombosed (clotted) and swollen and painful. They can also prolapse and extend out of the anal canal. Try sitting in a hot bath to relax the sphincter muscles. A visit to the doctor for anoscopy exam seems reasonable to rule out other conditions.

Get MD Rx: Internal hemorrhoids are not well treated by hot sitz bathes because of their internal location. A MD visit  can obtain Rx medication that can hopefully shrink the hemorrhoids. If that doesn't work, surgical intervention may be necessary as a foul smelling person is socially a pariah.

Unlikely: Probably more related to diet.  Colon cancer can occur at your age but common things are more likely.  If there is a family history of cancer, then go see someone.  Is there a free health clinic you can go to?  Could be a hemorrhoid  but more likely diet.  Hope this helps.

Fissure: Typically a rectal fissure is the source of severe rectal pain and bleeding. The hemorrhoids are less painful, cause bleeding, and often hide the fissure located under or deep to the hemorrhoid. Initial treatment is softer, smaller caliber stool with bowel regimen. If not getting better, then surgery consultation. Take a friend with you. Be well

Lower abdomen discomfort can be from constipation, urinary infection, gynecologic, etc. Needs an examination if not already gone to check for hemorrhoids or consider colonoscooy if you have not had one done before.

Possibly: See a proctologist for a careful rectal examination. If no rectal pathology is noted, have a prostate examination by a urologist to check for prostate congestion even if you have no urinary symptoms.
