No: Prostate enlargement is a normal occurence in men. It is hormonally controlled and cannot be delayed or prevented by diet or activity. There are medications available to help shrink the prostate but is reserved for older men who have urinary issues with known prostate enlargement.

urinary tract infection
prostatitis
prostate cancer

overactive bladder

Is risky: Sudafed has the exact opposite action of Alpha blockers which are effective in men with BPH and lower urinary symptoms. Sudafed use can increase the resting tone of the bladder neck and increase risk of urinary retention in men who may already be at some risk due to prostate enlargement. This scenario is not uncommon in the clinic.

Yes: Some antihistamines have been implicated in cases of urinary retention in men. A large prostate is per se a risk, and certain medications can precipitate this problem. However, the most potent risk is from decongestants like sudafed and phenylephrine which constrict the bladder neck and are often combined with antihistamine. Nonsedating medications like Claritin (loratadine) should carry less risk by itself.

: At age 77 a PSA of 3.9 is considered normal.  IT should be monitored with digital rectal exams and PSA.  Unless there is more rapid increase, do not be too concerned,

Not aware of any: I suppose a kid can get an infection in his prostate(which  might enlarge it), i'm not aware of condition affecting pre-pubertal males nor do I recall ever having read of one. A narrowed tract (xray) going through the area of the prostate in kids is more likely an abnormality of the pee channel, not enlargement of  the prostate as would be seen in adults.

Accepted Answer

Infection?: Enlargement of the prostate is usually thought of with bph, in older men, but can be seen (usually temporarily) with infection of inflammation of the prostate, like prostatitis.  In the younger male, this is usually related to sexual activity, and should be treated with antibiotics.

Prostate: You run a bigger risk of prostatitis rather than an enlarged prostate at your age.

: Urologists use various medications to stabilize bladder function, and tamsulosin may be useful.  Would contact the specialist to inform of the challenges and get advice as to where next to proceed.

: So far, the course of events and decisions by your doctor seem reasonable and appropriate.  But at your age of 38 yrs., suggest you be a bit proactive and see a urologist to cover all possibilities.
