What are they? : Is your son generally well?  Does he have anything other than pulsitile tinnitus? Is he small or diabetic? Does he have a history of head injury or brain infection?  These are just a few examples of the hundreds of questions that need to be answered for a pediatric neurologist or endocrinologist to follow up on this MRI finding.

Pituitary "enlarged": You need a ct or MRI to see the pituitary. The pituitary normally enlarges during pregnancy and puberty.  Hypothyroidism (high tsh) can make it enlarged.  Bad stuff: tumors in the pituitary can develop, and they sometime secrete excess hormones, giving rise to many different disorders.  Aneurisms, cysts, other things can happen, but all these are readily seen on mri.

Maybe nothing: There are physiologic reasons why the pituitary may swell, such as pregnancy, hypothyroidism, adolescence.  The questions i would ask: is there any hormone disorder? Is there any other abnormality noted on the mri? Was this a "pituitary mri"?  Did the radiologist misinterpret this? Many questions. See an endocrinologist, certainly before anything invasive is done.

Pituitary: There are many normal physiologic conditions that enlarge the pituitary. Pregnancy, lactation, hypothyroidism.  Presumably your sister had a ct or mri.  Did it show a tumor?  Just enlargement?  It would be important to have measurements of pituitary hormones to determine what the MRI means.  Prolactin, tsh, ft4. Is she menstruating? Normal menses suggest normal pituitary.

Several: It may mean you have a pituitary tumor, or a disorder known as empty sella syndrome -- but those questions are best answered by your doctor who has seen both you and the x=rays.

Very serious: These do not invade or destroy by infiltration like cancers do, but they are still extremely troublesome. 10 mm or more is a macroadenoma, but far smaller ones can ruin lives. If you are offered bromcriptine, give it a try and see if it helps. This often puts these tumors to sleep and lets you have a good life.

Small pituitary: F of unknown age asks: I have all the symptoms of Cushing's Disease, but my MRI shows a small pituitary gland. If my pituitary gland is small, why do I have a buffalo hump? ANS: unlike many other things size does not matter (much) with the pituitary. You need blood tests. Do others in your family have the hump? Google Cushings images to see if you look like them. If so blood/salva tests are needed

Hormones: Yes, it is possible to have a problem with the pituitary gland mimick the symptoms of hypothyroidism, such as fatigue, weight gain, lethargy, constipation.  There are several blood tests that assess the function of the pituitary gland and the hormones that are secreted.  Your physician can go over them in detail.

A bit tricky: The pituitary grows during childhood and peaks in the 2nd or 3rd decade.  Most studies measuring pituitary size on MRI focused only on the height of the pituitary gland and find that the mean peak height is usually about 6 mm +/- 1 mm.  More recent studies measure the actual volume of the pituitary with special 3d mri.   The mean volume for 5-9 yo girl in one study was 336+/-113 mm3.
