That is a very high: level and implies a hereditary condition or getting a lot of it. (many transfusions, a LOT of iron vitamins). Mostly, HEMOCROMATOSIS .. a hereditary condition. The iron builds up in the skin (gray , slate color) and liver. ANd can cause liver failure/cirrhosis/death.

Easy: 2 Diff Issues: Look up on net: ferritin=complex protein which stores iron atoms inside all cells. Serum iron=iron circulating via transferrin binding proteins in blood plasma. Like most/all lab/medical tests: just sample tiny pieces of what is happening at 1 instant; regard as only clues & ea prove nothing per-se. Mcv on cbc, se fe, s. Ferritin, TIBC (transferrin), bm bx, along with hx bld loss are common clues.

iron studies from 3/30 shows elevated ferritin 317 (16-228) tibc 488 (250-450) iron 113 (45-160) and saturation  23 (16-45). had 2 iron infusions in dec why would tibc and ferritin be elevated?  tibc was 654, ferritin 45 prior to iron?

: Ferritin rises after a transfusion - decreases over several weeks, it was ok before the infusion.  Arthritis/ hyperthyroidism / liver disease/hemochromatosis can increase ferritin.  Transferrin is high when there is iron deficiency anemia. It seems yours is coming down. What was your blood count  and red cell indices (MCHC ,MCV ) ,before the transfusion and what is it now? Check for thalassemia.

: It is a complex dynamic that you may not want to bother with because sometimes elevated ferritin may be because of inflammation or infection in the body. If your hemoglobin and hematocrit are normal and the shapes of the red blood cells are normal you should be contented.

I'm not concerned: Just having a mild viral infection can concentrate your blood and raise your serum ferritin slightly. I wasn't surprised to see this was the case when i opened your question.

i have hemochromatosis and recently i have been having really bad headaches x 3 weeks a vision more blurry. could this mean my ferritin level elevated?

May not relate: Hemochromatosis is a disease that almost always has few, if any neurological issues. The bad headaches may be due to multiple causes, but not ferritin changes.  Consider migraine, muscle contraction, eye issues, dental infections, even intracranial issues.  Best to have an mri, especially with constant issues, and worsening vision.  Check for increased pressure in head.

What's going on?: If you have an elevated serum ferritin, then you are almost certainly not iron-deficient and should not be receiving injectable iron. It can kill you. I am not joking. Please get with someone who knows how to work up an anemia properly. I'm betting on one of the milder thalassemia variants, but there are literally hundreds of other possibilities.

Donate blood: What you described is not necessarily hemochromatosis. There is a genetic for it that you may wish to get in consultation with your doctor. If you are otherwise healthy consider donating blood, it will reduce your iron and you will do a good deed. See this site for more info.
