Not always: Most cases of hairy leukoplakia are reported in HIV patients and is related to ebv infection. However, it has also been reported in patients without hiv. Patients with immune compromise (post-transplant) or autoimmune problems may develop hairy leukoplakia.

No: No, but if you truly have oral hairy leukoplakia (and not just a white coated tongue), then you should be tested for hiv.  Then again, everyone should be tested for hiv.

White patch: Leukoplakia is a condition seen in the oral cavity where white patches form on the oral mucosa  and the hairy form most often on the tongue.  Most often associated with EBV virus and not the HPV causing squamous Ca.  In the oral  cavity it is most often benign but still should be checked periodically

Many possibilities: Fortunately, the viability of HIV is very short lived outside of the body and is not contagious once exposed to air and surfaces that dry. Many other possibilities to causes for leukoplakia, best manner for a definitive diagnosis is with tissue biopsy.

Probably not OHL: I think you've been reading too much information on the internet, without real understanding. Several things that can cause various marks on the tongue in addition to hairy leukoplakia. Without HIV, OHL is a minor condition. Even with HIV, it's definitely not a "death sign". HIV is rare in 19 year olds in the US. even w/ at "hi risk. See doctor or dentist, and expect a reassuring response.

Biopsy it.: Any lesion is best diagnosed with an actual biopsy and not a visual guess.  Then you know exactly what you have and what precautions or treatment is indicated.  Don't be scared, just have it biopsied.  Personally, i think it's better to know than to worry about possibilities.  If your periodontist doesn't want to have it biopsied, ask why and consider seeing an oral surgeon for a second opinion.

Probably not: Hairy leukoplakia is more consistently related to, or caused by, the use of an oral antibiotic.
It can be treated by the use of topical application of 35% hydrogen peroxide or podophyllin. See a dentist or other professional who is familiar with this problem.

: Before worrying about these or any other causes, make sure you in fact have HL; other conditions can appear similar. See a doctor, preferably an expert in oral health such as your dentist or an ENT specialist, then take it from there.

: Very different (but look very similar). Oral leukoplakia is a pre-malignant process on the tongue that is easy to confuse with oral thrush.  Oral HAIRY leukoplakia is a benign lesion of the tongue often associated with EBV infection and HIV infection.  Oral thrush is a yeast infection - looks similar to the other 2 but quite different.
