Depends: Is it still a kidney stone of has it passed into the ureter and is now a ureteral stone? If it is just sitting in the kidney it will probably stay there for some indeterminate time, maybe, "forever". If in the ureter, it can cause pain and blood in the urine as it makes it way down.  The chances of stone passage  for stones 5-7 mm, 60%; for stones 7-9 mm, 48%.

Should be OK: Hope you r not obstructed and flying to get your stone treated by eswl (shockwave llithotripsy). Can always ask you dr. For prescripction for strong pain medication just in case u get osbstructed on the way. Good luck.

Yes: The persistent pain could be a sign that it is infected or in a position that would preclude it from passing on its own

No, but...: Changing altitude will not alter the odd for this 4-mm stone to move, but its timing of moving and dropping is entirely unpredictable so air pilots are advised to be stone-free. As to air passengers, it is up to their choice and risk. Besides, personal past history of stone disease may be a good reference to project how possible a stone in kidney may move, but still not 100% certain. Best wish ...

If stable: I wouldn't want to be cooped up on a plane while passing a renal stone but if there is a stone that has been happily residing in my kidney for some time there should be no issue with flying.

Renal stones: Small renal stones in lower pole calyces are less likely to spontaneously pass into the ureter.  If it did, you would likely pass it.  Generally safe to travel. If you were to be the solo pilot of a plane, i would have some concerns due to the severity of ureteral colic (despite the low probability).

Would not fly in pai: How long ago did stone pass? 4 M&amp;M should come out but you may need doctors help. Pain should be less unless stone us impacted (stuck) in ureter or urethra.

Get seen!: Kidney stone pain is horrible!  Go to an ER now and get help with the pain and help passing the stone.  Why suffer?

It is variable: The size of the stone is very important. The larger it is ,the less likely it will pass. If you have  severe pain or especially fever, you need to seek immediate medical attention.
