More information?: Complicated problem. First, depends on what you mean by "tmj". Minor open bites can be corrected with orthodontics. Major open bites need surgical correction. If you are having braces for this set of problems, you must make sure that your dealing with a board certified orthodontist with vast TMJ experience. Get a second opinion.

TMJ pain: TMJ pain is most often caused by chronic grinding and/or clenching your teeth, or a bad bite. In any case, diagnosis and treatment by a TMJ knowledgeable dentist is important. Painkillers will not eliminate the problem, which will continue unless the cause of the pain is eliminated.

Common sense!: Tmj problems often respond to simple conservative treatment. This includes a soft, non-chewing diet consisting of things like mashed potatoes or grits, and avoiding gum chewing. Also, many patients respond well to over the counter non-steroidal anti-inflammatory medications, like Ibuprofen or Naproxen (aleve). Another mainstay of treatment is moist heat applied to the joint.

Not Working: You still describe a disc that is displaced.  This just means that the arthroscopy treatment is not working.  Either the cause of your TMJ problem needs to be addressed, and/or another type of treatment attempted.  Unfortunately, this could mean a TMJ "surgery".

Pain in the muscles that support this joint, the teeth, or the joint itself, as well as referred pain can all occur. Different things: Depending on duration of pain, try cold compress (less useful if pain longer term), alternating heat and cold compress application, sleeping on your back, getting half an hour aerobic exercise per day, alternating NSAID meds with tylenol, (acetaminophen) avoiding gum chewing or really hard foods, and gentle stretching exercises. If no help from these, see a tmd aware dentist for the next diagnostic steps.
