Sl increased risk: Diclofenac is a NSAID which primarily are used as antiinflammatories and analgesic agents in patients with acute or chronic inflammation.   As a class, these meds carry a small absolute cardiac  risk increase primarily triggered by a 1-2% incidence of fluid retention or BP elevation (which can destabilize a cardiac patient.  For most this is not too high a risk and is offset by the benefits.

Increased: Studies have shown a 10-15% increased risk of heart attacks with nsaids (voltaren, motrin, Aleve type medications). One cannot determine an exact risk for an individual patient. Best is to take tylenol (acetaminophen) for pain, as there are no increased risks of heart attacks with it.

Specific question?: all the anti inflammatory meds (motrin etc) will have warnings for side effects not limited to the heart.
It is more for chronic daily use , if you are taking just a few days for your dysmenorrhea it is not without side effect potential but it is less.  that said is they are taking off market you have to change to another drug.  I recommend aleve (naproxen) (naprosyn) sparingly..GI, renal, BP AE also possibe

Tylenol (acetaminophen) or topical: Tylenol/acetaminophen has no effect on blood pressure. Also, there are topical antiinflammatories such as diclofenac gel/cream and others made by compounded pharmacies that have limited systemic absorption and rarely increase bp. Common sense with ri: Go down to the drugstore and check your blood pressure and pulse daily for a week without it. Write it down. (with your doctor's permission) and then do this while taking arthrotec (diclofenac and misoprostol). Then review it with your doctor. It could be nothing or a problem, depending on what your cardiac system is to begin with.

hi dr, i am taking diclofenac for my osteoarthritis and and heard that it is not safe, causes ulcers, high blood pressure and heart attacks, what is safe to use?

NSAIDs: NSAIDs  like diclofenac are valuable medications for patients with arthritis and other inflammatory conditions. The risk of ulcers and complications tends to be dose related. Therefore, the smallest effective dose of NSAIDs is taken to minimize the risk. Take NSAIDs with meals and talk to your doctor about receiving a special NSAIDs  called selective Cox-2 inhibitors.

i have heart disease and take ranexa, daily aspirin, statins. i had a wrist injury and taking naproxen and applying diclofenac gel. is it safe to take for over a week? i felt some palpitations in the last couple of days, so was wondering. thank you?

: There is no absolute contraindication unless you have chronic kidney disease or uncontrolled hypertension. However, using naproxen and aspirin together increases the risk of gastritis so you should take either omeprazole or famotidine for stomach protection until you stop naproxen. Wish you have a quick recovery.

Reversible: We use the terminology with respect to nuclear perfusion images of the heart muscle. When there is an image defect that does not fill in on follow up imaging, we call this nonreversible or fixed. It usually suggests scar of the myocardium rather than temporary decrease in blood flow to the area.
