See gynecologist : A dermoid cyst often does not cause pain but may need go be removed to rule out malignancy.  Benign cysts are often painful, at least intermittently.  You should be evaluated by a gynecologist and have a through evaluation and possible exploration.

Mature teratoma: It does not require removal at this size, although dermoids will need to be removed at sometime in the future. It is a benign tumor that can grow quite large over years, and is much more easily removed when small to mid-size. They do not "go away on their own." Many Gyn surgeons would remove it for you at this size: discuss with  doctor you trust what's best for you. Good wishes

Based on location: Not sure whether you refer to dermoid cyst of cranium or of ovary .In brain , based on the location and the symptoms the course of action -sy is decided. If  close to the brain stem region -lower part , potential for getting infected causing meningitis is there. Similarly  rupture can end up even fatally proceded by seizures, severe head ache. Maligant transformation is possible too. Consult sy/gyn.

Infection: If a sebaceous cyst ruptures, there is more risk that it will become infected.  Ideally, sebaceous cysts should usually be excised before they get so large that they rupture.  If they get infected, they should usually be surgically drained and packed with gauze to control the infection.  Antibiotics usually are needed. Surgical excision is often advisable after control of the infection.
