very good afternoon doctors. i have to take a fleet enema for a transperineal prostate biopsy. i am very uncomfortable about administering it myself..can i take an oral laxative like miralax or dulcolax like i did when i prepared for my colonoscopy?

: The type of bowel preparation needed for the different procedures is that found by your treatment team to work best in a given situation. A Fleet Enema is quite effective for outlet level clean out. MiraLAX or Dulcolax would not be as effective in general and may take several days of use to be even partially helpful. Discuss this with your doc. If there are options they will know the best ones.

No: Usually the small volume of a fleet's enema is not a problem. If this is a persistent need to use the enema for a bowel movement, then something other than simple constipation may be a problem and should be evaluated.

Constipation remedy: Need more details: have you had any prior bowel surgery or a hernia? If yes, check in with your PCP or surgeon. If not, then try 30 gms of fiber, 8 glasses of water, 4 oz of prune juice, 1 tablet of Peri-colace, 1 teaspoon of flaxseed oil or sesame oil with a glass of warm water with 1 teaspoon of lemon juice and  1 tablespoon of blackstrap molasses. You should have a bowel movement after this.

Fleets enema: commonly used ~4 oz saline laxative; sodium phosphate-based; can cause adverse reactions in certain individuals (age &gt;55) with certain pre-existing conditions (e.g. dehydration, impaired kidney function, congestive heart failure, abnormal electrolytes) if used too often (&gt;3 days or &gt;1 dose/24 hrs). I recommend you ask your MD about proper dosing and alternatives.

You may take a fiber laxative like Metamucil, drink enough water daily so that your urine is colorless. For the long term you may try to increase your intake of fiber foods. Consult this site for fiber foods.

Constipation: You could try one more enema about 24 hours after the first, but if that is not helpful, you need to see your doctor.You shouldn't be doing lots of self-medicating if you are not getting the results you anticipate, because you may have wrong diagnosis. Maybe your abdominal pain is not constipation at all but something else going on. You need a good history/physical.Then doc can recommend next step.

Yes: You can use a laxative once in a while but concentrate on eating healthy with enough fiber and more fruits and vegetables to avoid medications. If chronically constipated you may need doctors advice of how to tackle it with a stool softener which would be safer.
