i had bilateral total hip replacements 26 months ago. is antibiotic prophylaxis still recommended prior to routine dental cleanings?

Hip replacement: Although this topic has been debated for decades, the best person to answer it is the doctor who replaced the hips.  Many factors come to play like the condition of the patients immune system, and overall health history. I will premedicate my patients for at least five years, and defer to patients hip doctor after that.  Personally i had my hips replaced ten yrs ago and will premed for life.

my dentist says i should have dental prophylaxis every 6 months.is it convenient to do this so often? wouldnt  it damage gums and dental enamel?

Actually, no: Twice a year is average, some people need the hygiene visits more often. It does not damage gingiva or enamel. Leaving calculus will result in bone loss and periodontitis.

Rheumatic Heart: This changes every few years- definitely  check with your cardiologist to see what they recommend currently.

is there an alternate way to take prophylaxis for endocarditis during dental procedures? i am in the high risk category.- my dentist is not in for a week. i do not tolerate high doses of antibiotics, is it safe to split the dose to before and after?

: Yes. As long as you are covered with an antibiotic before as well as afterwards and the dental procedures are not that invasive. You could also consult your PCP until your dentist returns.

No extra precautions: Routine dental cleanings (prophylaxis) for children with sickle cell anemia are not different than dental prophylaxis for normal children. Because children are not sedated, the risk of decreased oxygenation is not there. For "operative" dental procedures such as tooth extractions and root canals, prophylactic antibiotics can be considered.

Yes: Amoxicillin , Keflex , Augmentin (amoxicillin and clavulanate)  ceclore  are all good for dental infections.

Not usually...but..: If asking about prophylactic antibiotics prior to dental tx, yes it is taken before dental work. This does not include needing to take it afterward for routine care. However, depending on the procedure, for example if you had surgery or something more invasive, your doctor may want to put you on the extra 5 days to prevent a post op infection. Consider indication for axbx and risk for infection.

rx'd clindamycin 150mg once every 6 hours for a dental infection, but that means staying up pretty late for the last nighttime dose. can the last nighttime dose be taken early so i can get a full night's sleep? if so, how early?

Schedule: Sorry. It's best to take the medication exactly every 6 hours. That maintains the proper titer. Taking it too soon or too late means that the level of antibiotic in your blood stream varies and therefore is less effective. And make sure to take it until all are gone. Dental infections require actual treatment, not just pills, so follow your Dentist's treatment plan.

was taking clarithromycin for dental infection but started throwing them up i'm now on clindamycin but throwing them up now too what should i do?

Tell rx'ing Dr.: Let the dr. Know you are also vomiting with the clinda, as it would be recommended to change class of antibiotics.  Best wishes.
