High haptoglobin is: Nonspecific. Probably just a marker of illness in general. In severe anemia oxygen delivery to tissue is compromised and lactate could rise.

my haptoglobin level is elevated, at 2.65 g/l (ref. 0.17-2.00). is this in and of itself a cause for concern (e.g. cancer)? i have also been experiencing low iron/hemoglobin/hematocrit for some time. kidney/liver function normal. gi tract is clear.

Most anything: The reference range, as usual, is set so that a few percent of healthies fall outside on either end. And if there's ongoing inflammation in the body, heptoglobin tends to go up. If you feel okay, I wouldn't concern yourself.

Yes possible anemia: In blood, haptoglobin (HG) binds free hemoglobin (Hb) (released from red blood cells) with high affinity and thus inhibits its oxidative activity. Therefore HG causes blood cells to lose their ability to carry and transmit oxygen to cells and tissues.  Almost like drowning at the celluar level.  HG is found in severe stress, burns, allergy and crush injuries.  Clinically low HG is c/w anemia.

Confirmation : Low haptoglobin confirms hemolysis, but it does not specify why the hemolysis is occurring. Additional would be needed.  There are many causes for hemolysis.

Need more info: What you described suggests a compensated hemolytic process in which your red cells have a shortened life span. It is not feasible to provide a meaningful opinion without additional history, physical examination and may be some tests. It would be prudent to see your doctor.

: In my estimation the only clinical value of testing haptoglobin levels is to determine the rate of red blood cell destruction as seen in hemolytic anemias.  High levels are seen in increased rates of the destruction of red blood cells which could be a secondary effect of many disorders that have high sedimentation rates. I don't think that is a prominent feature of fatty liver or gastroenteritis.

Monitor this: You need to continue follow-up with your doctor. Pathology may emerge over time, but many patients have slightly elevated lft's like yours, that never amount to any problem. In many cases, fatty infiltration of the liver is identified by ultrasound, and does not progress to actual liver disease.

can i possibly have hemolytic anemia with normal reticulocytes and ldh, but no haptoglobin, high mchc plus low alpha 2 globulin? currently i have itp and very low lymphocyte count. symptoms include shortness of breath, svt episodes, fatigue?

: A man with hemolytic anemia could certainly have symptoms of fatigue and even shortness of breath.  You will want to be sure your hematologist knows all of your symptoms and is sure your anemia is controlled.
