Urethritis: You need to be treated with antibiotics. You should see your pcp for a prescription. You should also be tested for other sti such as gonorrhea, syphillis and hiv.

Chlamydia: If you have a urine test that is positive for chlamydia, that is most likely real and you should get treated. Chlamydia is unlikely to show on a culture. The chlamydia test does not come back immediately positive on the day after exposure. the exposure most likely happened 2 or more weeks ago. You and your sexual contact from that time should get treated.

No: Both germs will never be found on a culture. They have special growth requirements &amp; are not tested that way. They can take pee and look for the DNA of either germ in that specimen, but it must be ordered specifically for that purpose.

: Almost by rule, doctor should discuss with patient about the condition and work-up result(s), how to treat and how to prevent the recurrence in the future if applicable. Because chlamydia is a STD so your doctor definitely has to inform and treat you and your sex partner should be treated as well.

: IgG, lgM indicate past infection. IgG late antibodies stay long term-may depend on repeat exposure ; IgM early and disappear after few weeks.  If concerned would repeat urine culture in 1-2  week but if  negative very likely you don't have active infection( unless recently re-infected and the number of chlamydia are low to detect.. F/U w/ MD

Maybe not infection: Since the question states that tests for infection have negative results, the cause of symptoms may be non-infectious. Such causes may include problems with the bladder muscles, the bladder or urethra lining, the rectum, the nerves that go to the bladder, etc... One's primary care doctor can do some more evaluation, and may refer to a urologist if needed.

Not an STD: Without urethral discharge, the chance of any STD is very low. STDs rarely cause such burning etc without discharge. Oral sex is particularly low risk, and virtually zero risk for chlamydia or mycoplasma. Any STD you might have had would have shown up with testing, and that it continued despite ciprofloxacin is additional evidence against infection. Almost certainly nothing serious. Maybe anxiety?
