when i cough there is pain in the left side of my chest like a rib is poking into my lung or flesh. i have been diagnosed with a collapsed lung before?

Recurrent: pneumothorax is definite possibility.  Other possibilities are pleurisy, pulmonary embolism, zoster, etc.  Get to your doctor, so he can get a chest x-ray.

cough for about 30 days,  chest x ray revealed partially collapsed lung (about 20%). just finished azithromycin.   pa at urgent care says i can fly?

Yes: Because of  commercial aircraft cabin pressurization systems and  20% or less is involved, it should be ok to fly. Many people with only one lung can fly without problems.  You may want  to wear a mask to protect your airway from new infections while healing from this one

have back, chest, shoulder, under breastbone pain with sob, mild cough, nausea for 6mo+. chest xray shows questionable infiltrate or pt. atelectasis. ?

It is complicated: You must have had Antibiotic treatment for Abnormal Chest Xray finding associated with the symptoms you described.
If these symptoms persists and Chest Xray does not improve you r doctor would recommend CT Scan of Chest and Pulmonary Function Test etc.
Nausea and loss of Appetite makes me wonder about abdominal problem also, 
Therefore Please follow your doctors advise.

SOB lying flat: Recent cold? or have another medical condition like HTN,DM, autoimmune disease, lyme,coxsackie?  d-dimer normal?Did CT check for PE? BNP level ?ankle swelling? What was pulm pressure?asthma?Rare  heart failure with normal ejection fraction. F/U w/cardiologist ask about use of  lasix (furosemide) (if ankle edema)or ,elevating  head of bed .

Abnormal chest xray : The chest xray should be reviewed and discussed with your asthma specialist. In general if you're asthma is stable and you have no lingering chest symptoms then the xray is non specific but should be able repeated in six months.

Poor Asthma Control: Sounds like you have a history of poorly controlled asthma. With 4 months of worsening cough and atelectasis on CXR I would recommend seeing a physician who specializes in managing asthma like an allergist or pulmonologist if not already done so in order to get better control of your symptoms.

have a chronic urge to cough 2 years. lung ct normal except for subsegmental atelectasis in right middle lobe and lingula. pft normal except for isolated reduced fef75 which was (70% predicted value). albuterol not helpful. what next steps, ideas?

: As opposed to linear atelectasis which is caused by diminished inspiration, subsegmental atelectasis is often caused by some obstruction in the airway leading th that segment of the lung. In this setting, bronchoscopic examination is frequently advised to determine the presence&amp; cause of obstruction. If you haven't consulted a pulmonologist, do so. A thoracic surgeon would be another specialist

No: Lungs may collapse for a variety of reasons, including trauma, blebs, bullae, increased pressure in the lungs, etc. While a pneumothorax may occur because of a bulla, it does not cause them.

Trauma or bleb: Most common cause is a rib fracture that punctures the lung causing lung collapse from pneumothorax. Another cause is rupture of a  bleb (an air filled blister on the surface of the lung)  if enough air leaks out of the bleb, the lung collapses from pneumothorax.  Depending on the size of the pneumothorax, a chest tube may need to be placed to remove the air leak and rexpand the lung.

No: A spontaneous collapsed lung happens when the lung tissue "pops" so to speak.  Growth spurts don't cause this but they are more common in tall thin people.
