Rash on Back: Without seeing the rash it is difficult to make a diagnosis.  However this could be tinea versicolor.
Tinea versicolor is fairly common. It is caused by a type of yeast fungus called pityrosporum ovale. This fungus is normally found on human skin. It only causes a problem in certain settings.

Dermatologist: If you haven't been examined by a dermatologist,see one. If you have, get a second opinion. I'd still be suspicious of some kind of insect bites or perhaps  pemphigus or pemphagoid reaction.

my scalp at the nape of the neck is quite itchy and i scratch it often. this is scalp covered by my hair. it now has small bumps on the skin. advice?

Could be ...: It very well could be a cause of more symptomatic seborrheic dermatitis or psoriasis, even a plain ole fungal infection.  While you could try over-the-counter dandruff shampoos like neutrogena T-gel, you would be best suited by first seeing your doctor to make sure it doesn't require more aggressive treatment. Wash daily: Try washing your hair and scalp daily with any shampoo. It may take about three months to see full effect  of this regimen.
For good health - Have a diet rich in fresh vegetables, fruits, whole grains, low fat milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex.

There are a number: of infectious and parasitic disorders that could present with these findings. This will not be able to be evaluated over the internet. You need to see a health care provider (probably best to see a dermatologist) for appropriate evaluation and treatment.

will rubbing nape of neck ( spinal cord area ) damage anything? i was rubbing the area because it felt super tense and stiff and i had rubbed what i thought was a muscle. could i hurt myself ?

question. i turned my neck to the right to try to look at my back area and i felt like i twisted it back too much. my neck had a sharp pain so i stopped twisting and looked straight. nape of neck feels tight. should i be okay?

: Probably. You likely pulled something in your neck and now it is a bit tight due to the reactive inflammation. Use Ice and Heat alternating and also ibuprofen/tylenol if you can tolerate those - 2 of each every 4 hours as needed. If not consistently getting better over the next 48 hours, see a doctor.

Tension : There could be a number of reasons, but a common one would be anxiety &amp; emotional tension that can cause headache through muscle spasm in the head &amp; neck. Also...Do you get enough sleep, drink enough water? Hope hat helps.
