However, depending on why you had the craniotomy and exactly where the vein is and what it looks like, there is a small chance that you need additional evaluation.  The best bet is to talk to your primary doctor, neurologist, or the surgeon who did the craniotomy.

Bulging Veins: Bulging veins in the hands and wrists are often associated with aging, being thin and well hydrated.  When the arms are down the blood in your veins have to work against gravity to return to the heart.  When you raise your arms the blood flows with gravity to move more quickly toward the heart.  Unlike the arteries which pulse blood, the movement of blood in the veins is more passive.

Vein or artery: Are you sure its a vein? If so, this may indicate nothing except that you are well hydrated. It could also mean higher than normal pressures in your chest, but this is unlikely. Given your age, its most likely just developmental. Follow up with your doctor if you remain concerned. If its an artery, it can be due to inflammation and your doctor may suggest some blood work.

Probably normal: These vessels can be more superficial particularly if you are skinny.  You should consider having your blood pressure checked to make sure it is in the normal range as high blood pressure can cause these symptoms - but otherwise you may be a little anxious.

Normal finding: The temporal arteries and temporal veins lie immediately adjacent to each other.  During exercise or increased activity, the arteries will pulsate because of increased heart rate and flow of blood.  What you are seeing is the arterial pulsation.  It looks like the vein pulsating because the vein sits over the artery and can move with the artery.
