Not a good technique: In the past, a few surgeons have used suture material to effectively enhance cup size as an alternative to breast implants.  The reason for this varied but generally had to do with the fact that the strings could increase the cup size far larger than the largest implants.  

At this point, i don't know of any reputable plastic surgeon performing this technique.

In certain cases: In certain cases, yes, insurance will pay for breast implants.  Specifically, patients who are undergoing reconstruction following breast cancer treatment, for instance.

All are safe: Both silicone and saline implants are safe. Neither increases the risk  of breast cancer. Silicone gel implants are gelatinous (gummy bear) implants that do not leak if ruptured.  They are more commonly used today. Liquid Silicone implants may rupture and leak, but leakage has not been clearly linked to subsequent disease.

Usually: Capsular contracture or scar tissue squeezing the implant.

No: Breast implants only need to be removed if there is a problem: significant capsular contracture, leakage, deflation, or rarely, infection. No problem, no removal necessary.

Aging Breast Implant: Breast implants also age.   That is why at some point in the future a woman should plan that the breast implants will have to be exchanged.  They can rupture requiring replacement or need to be exchanged and a lift added as the breasts sag.  Most women can go at least 10 years before needing a replacement.   However, some women can go 20-30 years without any issues.
