Could the spider: Of been hungry again? Your options inlcude having it treated a second time but instead of going to urgent care see a dermatologist or podiatrist. Take a picture in case the blister should look different when you go to the doctor. The doctor could opt for a biopsy which may shed light on if there is a fungal component or traumatic, or autoimmune etc....

Blister: Since there is itching it may be a bite or a very localized contact dermatitis such as poison ivy. At this time, keep the lesion clean,pat dry and apply a cortisone type cream to reduce itching. Spreading redness and warmth would suggest secondary bacterial infection which could need both topical and oral antibiotic. See MD as needed.

What caused them: Go see a doctor. Do not drain them at this time until the cause can be determined.  Did she burn her ankle? Did something sting her. Does she have underlying kidney issues? An evaluation is needed. See your doctor or a podiatrist.

Possible scar: If the most superficial layer of skin is involved in the blistering the chances of scaring are low, but the deeper the involvement in the skin the higher the chances of scaring.  If the pressure or shearing forces creating the blister are repetitive in the same area, then callus will form thickening the skin and changing its normal appearance.

Bites?: Insect bites in that location would be the most likely.  Try an antihistamine medication or cream for itching  and see your doctor if they do not resolve or spread.

It should fade: Usually the scarring from a blister resolves by itself. If you are really concerned, you can use Mederma scar treatment. You can pick this up over the counter in almost all of the pharmacies. It will take some time for this to go away.

Bad shoes: Blisters are the skin's reaction to high shearing forces. There is something that is rubbing on your skin causing them to develop. Correct the shoes will be helpful. Have to be careful with 2 pairs of socks or using Vaseline. They can make the problem worse when used in the wrong situation.

i got a blister on my ankle 2 days ago (from shoes). now my ankle is swollen though i never injured it, but the blister is red. do i need it checked?

Excellent idea: Could be swelling from infection and need antibiotic treatment. Until then, soak it in warm water, elevate it, clean it with mild soap and water.
