Could the spider: Of been hungry again? Your options inlcude having it treated a second time but instead of going to urgent care see a dermatologist or podiatrist. Take a picture in case the blister should look different when you go to the doctor. The doctor could opt for a biopsy which may shed light on if there is a fungal component or traumatic, or autoimmune etc....
