had a ct scan done said i have calcified granulomas bilateral hilar and and a subcarnial lymph node prominence pulm artery..should i be concerned?

Abnormal CT scan: The lymph nodes could be concerning for underlying pulmonary issues   ??? TB or other types of infections.   Why was the CT done??   -  I would circle back with whoever ordered the test  -   The pulm artery finding doesn't seem worrisome.    But the lymph node issue needs further investigation

i have bronchitis when i was young. i am smoking. and my recent x-ray found calcific granuloma on my upper right lung. what could cause my granuloma?

TB: The most likely cause would be healed tuberculosis in which case you should have a positive TB skin test or blood test such as quantiferon TB gold test.  If one of these is positive at your age wou would need treatment for latent tuberculosis.
Some fungal infections can also cause calcified granulomata in the lungs.

incidental: multiple bilateral sub 4 mm nodules, stable compared to the prior ct. small calcified granuloma in the left lower lobe. i did not have contrast. do i need follow up? how can they tell its not cancer and why do i have these?

From your description, a resolved infection is one likely condition to consider, including Coccidioidomycosis. If you have risk factors for lung cancer, blood marker testing may be helpful. Please follow up with your Pulmonary specialist. Watch for signs of shortness of breath and fever.

Mostly normal: These are all trivial imaging findings. The old stuff in your spleen probably reflects a long-ago victory over an infection with histoplasmosis. Kidney cysts are trivial and very common. The calcium stuff in your pelvis is probably phleboliths which never cause trouble. I hope the kidney stone stays in place or can be managed.

: Scarring is as a result of an old infection as is calcification. Punctate means tiny point like, nodule- solid area. Thias last thing is to be interpreted  and treated in co-relation with the reason the CT was ordered- by the ordering doc Good luck. ALWAYS ask the ordering doc to explain the reports of all tests ordered- to your satisfaction.

Incidental finding: On the MRI, they did not some soft tissue lesions with calcification in your subcutaneous tissues in the midline.   When they get an MRI for a superficial lesion they will often put a marker there to identify the area in question.  So presumably these lesions were not in the area of the palpable "mass" being evaluated.  I would discuss with your doctor and go through your MRI with him or her.

It is a benign: nodule, usually the result of a prior inflammatory process or infection.  It is typically an incidental finding in the lungs, but can be seen elsewhere.  Generally it is nothing to worry about.
