Incidental: multiple bilateral sub 4 mm nodules, stable compared to the prior ct. small calcified granuloma in the left lower lobe. i did not have contrast. do i need follow up? how can they tell its not cancer and why do i have these?

From your description, a resolved infection is one likely condition to consider, including Coccidioidomycosis. If you have risk factors for lung cancer, blood marker testing may be helpful.
