The PMI is the area on your chest where you feel the contraction of the heart the strongest (usually below the left nipple.

Apical is taken by listening to the apex of the heart in the chest wall. It may be more accurate when extra beats are present.

Take it at wrist: The apical pulse is taken at the apex of the heart. This is taken by using your stethoscope. The wrist pulse is just fine and is free. You do not need to buy a stethoscope. This will do just fine.

By location: The heart's location is sufficiently the same in the chest cavity of most adults that the pulsation from the tip/apex can be palpated/felt.Trace a line down the chest from the mid-point of the collar bone (mid-clavicular line) and then feel between the ribs. The apex of the heart is between the 5th and 6th ribs (5th intercostal space). An left parasternal lift, for example, is felt by the sternum!

Not sure: Most people with cardiomegaly do not feel the enlarged heart, but have other symptoms like leg swelling and shortness of breath.  That being said, a doctor can tell by a physical exasm and conform with an x-ray or echocardiogram if your heart is enlarged

why is my apical pulse much stronger and almost beating thru my chest when my heart rate is slow? because when it speeds up like 80 bpm you don't feel it??

Pulse: When your heart is slow there is more time to fill and each stroke may have a larger volume of blood explaining why you're feeling it. With the higher rate, there isn't as much time to fill so each stroke's volume may be lower and not felt as much.
