Angiomyolipoma Dx: The addition of MRI to rule out angiomyolipoma helps a little. It is also a good way to follow the lesion to see if it grows (no radiation and the dye is safer).
Chronic lung changes in a 35 yo is nonspecific and needs clinical correlation. Some folks may do breathing tests if there are symptoms like shortness of breath, even if only for baseline. Everyone should quit smoking ASAP.

Benign tumor: This is a fairly common benign tumor. If the imaging specialist is certain of the diagnosis, it's perhaps best left alone. If you have more than one, or other signs, it's suggestive of tuberous sclerosis, a highly variable genetic illness. A big one may respond to some recently-devised medical management.

Depends on reason: Embolization is indicated for renal angiomyolipomas in the setting of bleeding from the tumors.  In such cases, the procedure can be life-saving and can also help preserve kidney tissue (as opposed to taking the whole organ out).  In skilled hands, it is considered to be relatively safe and necessary procedure.  As with all vascular procedures, the major risk is bleeding from the site.

Benign tumor: They are most common benign tumor of kidney. Comprised of vascular cells, smooth muscle cells ; fat cells  they are liable to spontaneous hemorrhage. Most commonly seen in patients with tuberous sclerosis. Usually do not require intervention. Optimal management is conservative to preserve as much normal functioning kidney tissue as possible. Large tumors can often be managed by embolisation.

Can be done: Angiomyolipoma or AML is a fatty tumor often considered a benign disease.  It can be solitary or part of a genetic complex called tuberous sclerosis and present with many deposits.  It carries a risk of bleeding.  Embolization or placing coils into the small arteries feeding the mass is one way to treat it.  The decision may require careful evaluation of the size and location of the mass.

Small lesion: 4 mm is very small lesion and differentiation from cyst or other tumor difficult. Angiomyolipomas  are benign and composed of variable amounts of three components; blood vessels (-angio), plump spindle cells (-myo) and adipose tissue (-lipo).If fat content recognized on ct or mr tumors likely to be angiomyoliipomas. Other tumors can have some fat content.

Angiolipoma: 22 mm is 2 cm which is moderate size. angiolipomas are benign  so would just monitor especially since there are not symptoms and surgery is not without risk .

Angiomyolipoma: These are a type of BENIGN renal neoplasm.  So yes it is a cancer but it is a benign form.   These are the most common benign solid kidney tumor.  Most of the cases are random events typically in females and around 40 years old.   The other 20% of cases have an association with certain neurological disorders.   They may cause flank pain, hematuria and can spontaneously rupture causing bleeding.
