Usually : Usually the hospital has copies of most every paper you were given. Please contact the hospital where you were admitted to request a copy. There may or may not be a charge for this.

i am concerned my formula fed 2 wk old hasn't gained weight since hospital discharge! ped is monitoring, should i be concerned about health issues?

Probably not.: Since your pediatrician is monitoring your baby, most likely there will be a follow up visit in a few days... If not, then call and schedule another weight check. In my 30 years of pediatric practice with thousands of newborn visits, i can recall only a handful of mothers worried about over feeding. Yet, many mothers express concerns of "poor weight gain" or "poor feeding". Have your baby weighed.

thyroidectomy. it is my 6th day in the hospital.  kept me for low levels of calcium. drs won't discharge me. why they won't let me go home ? and is it life threatening to sign out against medical advice?

Do not sign out AMA: Hypothyroidism can be life threatening.   This is a potential complication and is monitored after thyroidectomy in case the parathyroid was either injured or removed during surgery.

Er discharge: Many patients are discharged from the er while still having pain, from an injury for example. However their overall condition and vital signs must be stable and the patient should not be at high risk for falling and, in my opinion, not objecting to disharge. A prescription for a pain reliever is often supplied by the emergency room physician with instructions for appropriate follow-up care.

Yes: Cough is a protective mechanism that helps mobilize mucus and debris from deep in the lungs so it can be brought out. It can persist for a month. You should use their other parameters of feeding/play/ etc. to decide if they need to be re-evaluated.

: This would be an unheard of event and unprofessional. In the US. an investigation and litigation would result. The hospital should contact the police, or put this persons profile in the papers or on social media and TV as a missing person. All human efforts should be used to help this person.

D/c planning: Discharge planning starts when the patient is admitted.  This includes social workers involvement to facilitate discharge to an appropriate place, home, nursing home or home health is needed. Whether the patient needs any equipment on discharge like wheelchairs, commodes etc.
