Pain: The biggest difference would be pain. Herpes in the oral cavity hurts, usually quite severely. Fordyce granules are always there and do not cause a problem. Herpes lesion develop from previous normal lining of the mouth. Frost e granules are always there.

Differences: Fordyce spots are enlarged oil glands & a completely normal part of anatomy. They are bumps found on edge of lips (whitish/yellow) that are usually 1 to 3 mm in diameter. They don’t hurt. For Herpes there is a cluster of lesions that look like blisters. They can spread & coalesce followed by crusting over.
