Is very very late syphilis hard to test for? i know rpr/vdrl can be negative, but will trep tests (cia/eia/tppa/fta) still be positive? can anything affect them and turn them negative? can sensitivity be low due to waning/fewer antibodies?

If both nontreponemal (VDRL/RPR) and treponemal (FTA/ABS) tests are negative, chance of syphilis (whatever stage) is very unlikely. If you have symptoms which suggest manifestation of late syphilis you should see an ID doctor for ridding your concern. Take care!
