If rashes are present and syphilis is suspected as the cause, nontreponemal test (RPR or VDRL) should be positive. If nontreponemal test is negative, syphilis is ruled out. If nontreponemal test is positive but treponemal test is negative, syphilis is also ruled out. If syphilis is not treated correctly and active disease (primary and secondary) is present, nontreponemal test should be positive.
