Random and non persistent testicular pain with normal testis and scrotum is nothing to concern and etiology is difficult to define. Gravity may be a precipitating factor so you can pay attention if that symptom usually occurs when walking or standing; if so, use underwear with athletic support as a trial. You should only concern if pain is persistent or getting worse.

Sometimes its due to a small upper pole epidydimal cyst which is often missed. It causes no other problem except can give discomfort when the testicle is in certain positions and it rubs against something. Best is to wear a good scrotal support and gently change the postion of the testicle when you feel discomfort. Good luck
