Per Se, an elevated LDL does not cause chest pain. The latter should be evaluated to see whether it is due to narrowing of the arteries supplying the heart with blood. It is the insufficient supply of blood to keep up with the demands of heart muscle that causes a specific kind of pain. A high cholesterol is one of the factors that may lead to narrowing of the (coronary) arteries.
