Likely lymph nodes: There are lymph nodes in the armpit that may become noticeable when a skin irritation or infection involves that shoulder or arm. I would not automatically assume it has to do with the breast surgery/issue. Your doc can evaluate & decide if this is important.

Need exam: You would need to see your doctor and have an exam to be sure. They may want to do an ultrasound if they think it is concerning.
