Concern?: How long has it been there? Trauma? Infection? Cyst? Fluid? The biggest concern would be cancer but you are a bit old for that but you should get it checked out.

Needs examination: As the other 2 have said, need to work out whether cyst/hydrocele, infection, hernia or very small chance of being something more nasty like cancer or other problem. Needs urgent examination with your doctor and you may need referral for ultrasound to fully sort it out if your doctor remains unsure after examining you. All the best
