See a doctor: hernia will not cause a lump on the testicles, it might cause a bulge in the groin in case it is a direct hernia, or part of the intestine or fluid might leak to the scrotum and cause a large scrotum. If there is a confusion about a lump in the testicle, you better see a surgeon for proper evaluation.
