Could be a fungus: Your skin could have a fungal infection, ie jock itch or something similar. Or you could be having an allergic reaction to something left behind from washing your underwear. Something new in the water supply or soap that you use is also possible. Time to call your doc for more help.

See your DR.: Not all std infections will be detected by blood tests. Not all symptoms on genital area are std. Chlamydia, ghonnorhea, hpv, and others are not found with blood tests. Phimosis of uncircumcised men is not std.
