Bad breath: Chronic bad breath isolated between teeth indicates a periodontal (gum) infection, a tooth infection, or sub-gingival decay. See your dentist or a periodontist for a thorough exam and dental cleaning.

Infection: Either teeth or gums or both are infected. Get to your Dentist to fix this before severe permanent damage is done.

Bad Breath: There are many causes of what you describe and the only way to determine the cause followed by appropriate treatment is to see a local dentist for a thorough medical history, clinical exam and x-rays. Causes may include dental decay, periodontal abscess, food impaction, improper or inadequate tooth brushing and flossing, fractured or loose filling, etc. not to mention some medical conditions.
