Two wks wiggle room: The effect of Depotprovera 150 mg every 12 weeks should actually last up to two weeks after you are due for the next depot shot - however, this is not certain. Before the next, delayed, depot shot women in your situation should not have sex for a week, then take a pregnancy test, and if negative, then get the depot shot.
