Rarely: Most cataract surgery is performed under local anesthesia, sometimes with a little assist intravenously but not asleep. The only patients consistently put to sleep are children, and adults with movement disorders and mental deficiency;.

Not usually: Topical eye drops are used as the anesthesia for most cataract surgery. If the patient is unable to hold still, local anesthesia may be used with IV sedation. The local is usually peribulbar now because it is safer than retrobulbar injection. General anesthesia (being put under) is reserved for extreme cases such as mental retardation. General anethesia is not usually worth the risk.
