Serious...yes: The hydronephrosis part is what makes this serious. The stone is basically plugging the urine outlet and the kidney is suffering. You need to see a urologist and have lithotripsy to break up the stone. At 7 mm it is unlikely to pass on its own. Otherwise you may loose function in that kidney and since you already have some scarring, your kidney function reserve is dwindling.
