MCL injury: I'd be concerned about an mcl sprain or a piece of broken off cement. The cement can be seen on x-ray. The mcl sprain/injury can be diagnosed on physical exam or mri, and can be treated with a brace, usually with full range of motion allowed.

Scar tissue: This is still remaining iritated tissue within the knee. Keep up your exercise and range of motion and this will eventually go away. Use topical antiinflammatory gels or liquid to apply where to have pain. It is not pain that indicates and problem with your prosthesis.
