If someone had inflammation or swelling of the brain. how severe would the headaches be?

Different things.: "inflammation" of teh brain might be painless, and present with seizures, paralysis, or altered state of consciousness. "swelling" presses against the dura (bag in which the brain sits) and that tends to become the worst headache ever - which is mainly how we find out about it.
