Eye lense: A cataract is a loss of transparency of the natural lens of the eye. Most begin due to aging but some are present at birth, some due to acquired disease, some due to drugs and chemotherapy and some due to trauma. But all are in the lense of the eye.

In the lens: there is something in the eye called the lens, and like the lens of a pair of eyeglasses, when we are born it is supposed to be crystal clear. Over time it becomes cloudy and that's when you call it a cataract. There are different stages of cataracts that are mild to moderate to severe. When cataracts are visually significant and inhibit regular activities, one would suggest taking it out.
