It depends: Like many things in medicine, it depends. Some of it depends on the patient's symptoms, age. Many people have "white spots" on mris; there can be non-specific small vessel white matter disease seen on the MRI of many individuals with a western diet. White spots on an MRI also can mean ms, but it is usually in a certain pattern that is more specific for ms. Discuss your MRI with your neurologist.

Multiple sclerosis: Multiple white spots on an MRI can be a sign of multiple sclerosis but additional clinical findings of ms would also be needed to made the diagnosis.

Not necessarily: Could be due to prior trauma, hypertension, smoking, aging, prior cocaine or amphetamine usage, aging or hardening of arteries, small strokes, etc, and not necessarily related to MS at all.
