Their dentist: Your child's dentist can easily polish off any external staining. If you've never taken your child to the dentist, this is an excellent time to start, especially at four years old.

Pedodontist: The staining might be coincidental since penicillin doesn't stain teeth. Have a pediatric dentist take a look. Your child might just need a prophy.

Stained Teeth: The penicillin is not causing the stain. Likely the four year old has a chromagenic bacterial discoloration or another medication staining the teeth on the outside. Are you rinsing with Chlorhexidine perhaps...That causes a temporary brown stain. The stain can be polished off at the dental office if so.

External stain: Antibiotics can discolor teeth during their formation, not after they have already formed or erupted...Could possibly be stain from food or drink, that will likely come off at a hygiene visit...
