Probably smaller: After the cast is removed, the ankle swelling should be down and the limb may appear smaller because of the immobilization and disuse atrophy that occurs in the muscles. It doesn't stay that way once you start moving again.

Atrophied: After a cast for an injured ankle is removed, the affected leg is usually thinner and the muscles are weakened. This is called atrophy. It will usually respond to physical therapy and rehabilitation.
