Tardive dyskinesia : This hyperkinetic movement disorder appears with a delayed onset, after prolonged use of Dopamine receptor blocking agents -- both antipsychotic and antinausea drugs. With first generation agents the annual incidence is 3 to 8% -- up to 20% in the elderly. It's less with newer drugs: ~2.1 % by averaging studies, higher (~5%) in elderly. http://tinyurl.com/qdneaoa it's very rare with clozaril (clozapine).

Yes to all: All the drugs you list can cause tardive dyskinesia. The incidence is 5% per year of drug exposure. The longer you take the drug, the higher the incidence. About 2% is severe. It can be permanent: stopping the drug may not reverse it.
