Yes: Even benign cysts when large enough can cause pain do to structural weakness.

Unlikely: Coccyx pain is lower & pinpoint on a rectal exam. The cyst may refer pain lower than it's located tho. See a spine surgeon 4 exam & imaging. A mr will aide in the dx, but most sacral cysts r benign, and have been there 4 some time.
