Hypoglycemia: Make sure his blood sugars are normal during this episode. This could be sign of low blood sugar. It may accompany by sweating and weakness. Also very low heart rate sick sinus syndrome or very high and irregular heart rate atrial fibrillation and also present like this. Needless to say he needs to see a cardiologist.

Take him to doctor: As you already described , he had diabetes , heart disease, most likely they are not under control , he needs tests and treatments , take him o his doctor now, he needs help.
