Chiari Malformation: Most benign syringes (plural for syrinx) are caused by a chiari i malformation. First a tumor must be ruled out with a contrast enhanced MRI of the spine. If the cavity is getting worse and there is no tumor, i would look into a chiari decompression procedure.

Would follow: Would follow the size of the syrinx with serial mri's. Also make certain that there is not a chiari malformation or tethered cord. Would consider physical therapy to start.
